      Background InformationAlpha-methylacyl-CoA racemase is encoded by the AMACR gene and catalyzes the convertion of various (2R)-methylacyl-CoA esters to the corresponding (2S)-methylacyl-CoA epimers. AMACR deficiency (AMACRD) is a rare autosomal recessive peroxisomal disorder characterized by elevated plasma concentrations of pristanic acid C27-bile-acid intermediates, resulting in variable neurodegenerative symptoms affecting the central and peripheral nervous systems, including seizures, visual failure, sensorimotor neuropathy, spasticity, migraine, and white matter hyperintensities on brain imaging.
