Chemicals subject to United States Department of Health and Human Services, Centers of Disease Control and Prevention (CDC) Select Agent regulations.

This list includes Enterotoxin Type B, Neurotoxin Type B, Tetrodotoxin Fugu sp. and Tetrodotoxin, Citrate Free Fugu sp. The regulations require that entities processing listed CDC select agents and toxins must register with the CDC and demonstrate compliance with specific safety and security standards for handling select agents and toxins. Any entity that possesses a select agent and/or toxin, unless specifically exempted is affected by the regulation.

A CDC Select Agent Declaration and End-Use Certificate will be required to release a sales order. An authorized individual at your facility who is familiar with the 42 CFR Part 73 regulations should complete and sign this form. If your facility is subject to the CDC Select Agent registration requirements, a copy of the Facility Registration form and an initiated Report of Transfer of Select Agents and Toxins – CDC form 2 must accompany the completed CDC Select Agent Declaration and End-Use Certificate.
