A copy of a photo ID of the purchaser issued by the Government of Canada or a municipal or provincial government must be provided. The form will be valid for one (1) year from the date of completion. The NRC regulations exempt certain entities from these regulations.

If you are ordering this material on behalf of a laboratory affiliated with a hospital or health clinic, a post secondary educational institution, or a federal or provincial government agency please indicate this and identify your company as required. If you purchase this material for further distribution, you must be enrolled as a seller with the NRC. Please attach proof of your enrollment.
